UC Student Health Insurance Claims 

For claims from Fall 2021, Spring 2022, and Summer 2022 will be processed through HealthSmart.

You can create an online account at HealthSmart to receive electronic copies of your explanation of benefits, and to also review your accounts.  If you did not provide your insurance information at the time of your visit, you may need to complete a claim form.

Need to submit a claim?  Complete the Major Medical Claim form and submit to the address on the form.  For additional assistance with claims, please call Health Smart at 1-800-331-1096.

For claims for Fall 2022 and forward United Healthcare will be your claims processor.

Manage your account online through United Healthcare Student Resources.  Get access to claims status, EOBs, network providers, correspondence and coverage information by logging into My Account.  If you do not have an account, you can create one by using your 7 digit insurance ID number, and clicking on Create Account.

In order for claims information to be discussed between our office or a guardian/family member and the United Healthcare claims office, a PRA must be submitted via your UHCSR account. Please see the steps to submit a PRA (Personal Representative Appointment).
